Once we process your form, we will ship your device and send you a confirmation email.
Once you receive your device, you will need to pair it to your myChart account. If you don’t have a myChart account, you can create one here or we can assist you over the phone: (877) 696-3622. Once paired, you can schedule your visit in the 24/7 Virtual Urgent Care or with select PCPs and Pediatricians via myChart (more providers are being added regularly!)
